Form Filler by Accuvoice Inc allows callers to leave complete and detailed information in response to prompted questions.

When the question is answered, another question is asked until all the questions have been asked and answered.
 
This is different than a voice mail system, which asks you all the questions at once and then asks you to provide answers for all of them, without providing you the ability to listen to the question again.
 
Transcribers can utilize any telephone when calling into the system. Once they have dialed into the system, they are asked to enter their PIN number, and then step through answers to each question on the call. With each answer in its proper place, a transcriber can rapidly complete the task.
